A dental implant is a small titanium post placed into the jawbone where a tooth root used to be. Over the following months the bone grows onto its surface and holds it firmly, a process called osseointegration. Once that has happened, a connector (the abutment) and a custom-made crown are fitted on top. The finished tooth is fixed, is cleaned like a natural tooth, and does not rely on the teeth either side for support.
The reason implants matter is what happens beneath the gum. When a tooth is lost, the bone that held it gradually shrinks, because bone needs the load of a root to maintain itself. An implant is the only replacement that sits in the bone itself, which is why it helps preserve the bone around the gap. That is a functional argument, not a cosmetic one, and it is the reason we take the assessment seriously.

Most adults with good general health, healthy gums and enough bone are candidates. Age on its own is rarely the deciding factor. What matters more is the health of the mouth and the body the implant will live in.

A well-planned implant in a healthy, well-maintained mouth can last for many years, and the literature describes high long-term success for carefully selected cases. Most adults with healthy gums, enough bone and stable general health are suitable. Active gum disease, heavy smoking, poorly controlled diabetes and some bone medicines raise the risk. Thin bone can often be rebuilt with grafting. Dr Rishil Patel assesses each of these at consultation and tells you plainly whether an implant is sensible in your case.
The placement itself is carried out under local anaesthetic, so you should feel pressure rather than pain. For anxious patients or longer procedures, sedation can be arranged where appropriate, and is discussed at your assessment. Most patients describe the next few days as tender rather than painful and manage with ordinary painkillers. Swelling usually settles within a week. For a single implant, allow roughly three to six months from placement to the final crown, because the bone needs time to integrate with the implant. Grafting or gum treatment can add time. The surgical appointment itself is usually around an hour for one implant, and the crown is fitted over one or two further visits.
Not always. An implant does not involve the neighbouring teeth and helps preserve bone, but it needs surgery and several months. A bridge is quicker and avoids surgery, but the teeth either side must be prepared, a poor trade if they are healthy. Where those teeth already need crowns, a bridge can be the more sensible choice.

Not while it is active. Gum disease raises the risk of peri-implantitis, which can cost an implant. We treat the gums first, usually with periodontal treatment and regular hygiene visits, and plan the implant once they are stable. Many patients who start with gum problems go on to have implants successfully once their gums are healthy.
Thin or shallow bone is common where a tooth has been missing for some time. It rarely rules implants out: bone grafting, and in the upper back jaw a sinus lift, can often rebuild enough volume. Grafting is quoted in writing after assessment. Where grafting is not sensible, a bridge or denture may be the better answer.
