What we can see today, when NHS 111 is the better call, and how to recognise an A&E situation, explained plainly by a clinician-led practice on Essex Road.
Emergency dental care at Dental & Wellness London in Islington means a same-day appointment, Monday to Friday 8am–7pm (Tuesday from 9am), where the day allows. This page sets out what we can see on the day, what an emergency appointment involves, what it costs, and where our scope ends. We would rather be clear about that than promise a service we do not run.
Most urgent dental problems fall into a small number of presentations, and our same-day appointments are structured around them. The aim of the first visit is simple: find the cause, relieve the pain, make the tooth safe, and give you a written plan for anything that needs more time.
If you are unsure whether your problem counts as urgent, call anyway. A two-minute triage conversation is usually enough to tell whether you need to be seen today, can safely wait for a routine visit, or need a different service altogether.
Phone first, WhatsApp second. Both are answered during clinic hours. We will ask when the problem started, where the pain is, what makes it worse and whether you have any swelling or fever, then offer the earliest appointment available. If we cannot see you the same day, we say so immediately and point you to the right alternative rather than keep you waiting.
Call NHS 111. Its dental pathway can give advice and, where available, refer you to an out-of-hours dental service. We are not a 24-hour clinic. Messages sent to us out of hours are answered at the start of the next working day, and where it is clinically appropriate we will hold a slot in the Monday morning book.
Go to A&E for significant facial swelling, especially swelling spreading towards the eye or down into the neck, difficulty breathing or swallowing, a suspected broken jaw, or heavy bleeding that will not stop with firm pressure. The Whittington, the Royal Free and the Royal London are the nearest hospitals from Islington. For anything life-threatening, call 999.

Our emergency appointments suit adults and children who are in pain or have damaged a tooth during the working week, whether you are a registered patient or not. Many people who call are new to us: they live or work near Angel, Highbury or Canonbury and need a dentist today rather than in three weeks.
It is not the right route for life-threatening symptoms, which belong with A&E or 999, or for problems that arise at night or over the weekend, which belong with NHS 111. It is also not the right setting for major facial trauma, which needs a hospital maxillofacial team. We are a private practice and do not provide NHS treatment.
Most people leave an emergency appointment out of acute pain, or with pain that is clearly settling, and with the tooth protected. How long that first repair lasts depends on what was done. A temporary dressing is designed to last weeks, not years, so the follow-up matters. A well-placed permanent filling or bonded repair can last many years with good care. Results vary from person to person.
After treatment we give you written aftercare: what to eat for the first day, how to manage any tenderness, when to take pain relief, and which signs mean you should call us back, such as swelling returning or a raised temperature. Many emergencies are the late stage of a slow problem: a cracked filling, decay below an old restoration, or gum disease left unchecked. Once the urgent issue is dealt with, a full dental examination and regular visits to our hygienist in Islington are the most reliable way to stop the next one. If night-time clenching is cracking teeth, we discuss teeth grinding and protective splints at a calm, routine appointment rather than under pressure.
The main risk in emergency dentistry is treating the symptom and missing the cause. We reduce it by examining the whole area, taking x-rays where they are justified, and not committing to irreversible treatment until the diagnosis is clear. The second risk is overtreatment under pressure. We separate pain relief on the day from definitive treatment, so that larger decisions, such as whether a tooth can be saved, are made with time to think and a written estimate in hand.
Any procedure carries some risk, including post-operative tenderness, infection or a restoration that needs replacing. We explain the specific risks for your treatment before we begin, and we tell you plainly when a problem is better managed by your GP, a hospital team or a specialist.

Not for toothache alone. A&E is for swelling affecting your eye, breathing or swallowing, a suspected jaw fracture, uncontrolled bleeding or signs of sepsis such as high fever and confusion. For ordinary toothache, call us during hours or NHS 111 out of hours.
Toothache becomes urgent when it stops responding to pain relief, keeps you awake, comes with swelling or a bad taste, or follows an injury. Mild sensitivity to cold that fades quickly is usually not an emergency, though it is worth having checked. Our sensitive teeth page explains the difference.

Pick the tooth up by the crown, not the root. If it is dirty, rinse it briefly in milk without scrubbing. If you can, put it back in the socket and bite gently on a clean cloth. If not, keep it in milk and call us immediately. The chance of saving the tooth is best within the first hour.
In most cases, yes. We find the source of the infection, relieve pressure where appropriate and start treatment on the tooth itself. Antibiotics are used only where they are clinically indicated. If swelling is spreading towards your eye or neck, or you have difficulty swallowing or breathing, go to A&E without delay.
Not always. Some toothache settles with a filling or dressing; other teeth need root canal treatment to save them. If so, we book it as a separate appointment with a written estimate.
