For the first 4 hours after a lip flip, stay upright, avoid vigorous exercise, and avoid touching or massaging the lip area. For 24 hours, no facials and no significant heat exposure. For 2 weeks, no dental work at the upper lip and no aggressive lip massage. Effect develops over 7-14 days; complimentary review at 2 weeks confirms outcome.
What this page is for
You've just had a lip flip (or you're booked in and want to know what aftercare involves) and you want practical, clear, day-by-day information about what to do and what to avoid. This page is that guide.
The aftercare for a lip flip is light compared to many aesthetic treatments — there's no significant downtime, no specific medications to take, no aggressive restrictions on normal activity. But the few things that matter, matter genuinely — they protect the precision of the result and reduce the chances of avoidable side effects.
For the broader treatment guide, see Lip Flip London. For the results timeline specifically, see Lip Flip Results Timeline. For side effects, see Lip Flip Side Effects.
The 4-hour window — most important rules
The first 4 hours after a lip flip is when the medicine is settling into the targeted muscle. Disrupting that settlement is the single biggest way patients accidentally affect their own outcome.
Stay upright. Don't lie flat for 4 hours. Don't lean over a desk for prolonged periods. Don't bend forward repeatedly. The reason: medicine can migrate slightly with gravity if the patient is positioned with the treated area pointing downward for sustained periods, and unwanted migration to neighbouring muscles is the mechanism behind several of the avoidable lip flip side effects.
No vigorous exercise. Cardio, weight training, hot yoga — all best avoided for 4 hours. Increased circulation during heavy exercise can encourage the medicine to spread beyond the intended target area.
No facials, facial massage, or pressure on the lip area. Don't deliberately touch the treated area. Don't apply pressure. Don't have someone else massage your face. The medicine is still settling and pressure can move it.
Stay hydrated but no straws or aggressive sucking motions. Normal drinking is fine; using a straw or any strong sucking motion engages exactly the muscle that was just treated and isn't ideal during the settlement window.
Skip makeup on the lip area for now. You can wear eye makeup; lip makeup can wait until tomorrow.
These 4-hour rules are the most important ones. Patients who follow them have meaningfully fewer issues with unwanted spread effects than patients who don't.

The 24-hour window — additional considerations
Once you're past the 4-hour critical window, restrictions ease. The next 20 hours still have a few considerations:
No significant heat exposure. Saunas, steam rooms, hot tubs, hot yoga, very hot showers (lukewarm is fine) — best avoided for 24 hours. The reason: significant heat increases circulation in facial tissue and can affect how the medicine settles.
Sleep on your back or with head slightly elevated tonight. Side-sleeping pressing the treated area into a pillow is best avoided for the first night. Back-sleeping or sleeping with an extra pillow to elevate the head reduces lip-area pressure.
No alcohol for 24 hours. Alcohol's circulation effects and potential interaction with bruising/swelling are reasons to defer drinking alcohol for at least the first day. After 24 hours, normal consumption is fine.
Eat normally. Soft foods aren't required. The lips will function normally for eating. Just avoid biting hard food in a way that significantly engages the treated area for the first day.
No dental work today or tomorrow. Any dental appointment that involves pressure on the upper lip area should be deferred for at least 48 hours, ideally a week. If you have a dental emergency, mention the recent lip flip to your dentist.
The 1-week window — when restrictions ease
By 24 hours post-treatment, most restrictions have lifted. The remaining considerations for the first week:
No aggressive lip massage or facial treatments at the treated area. This includes massage facials, professional facials with significant lip-area pressure, microdermabrasion at the perioral region, and aggressive home facial massage. Standard skincare with light application is fine.
No dental work involving upper-lip pressure. Routine dental checkups, hygienist appointments — fine after 48 hours. Significant dental work involving sustained upper-lip retraction or pressure (extensive bonding, anterior crown work, etc) is better deferred for 1-2 weeks if possible.
Sun exposure is fine. Standard sun protection applies; the lip flip doesn't change skin photosensitivity.
Exercise normally from day 2 onwards. No restrictions on cardio, weight training, yoga, etc beyond 24 hours.
Lip makeup from day 2 onwards. Lipstick, lip balm, lip stain — all fine from the morning after treatment.
Kissing is fine. Reasonable question that patients sometimes feel awkward asking. Kissing isn't a problem at any point after the 4-hour window.

The 2-week window — peak effect and review
The lip flip effect develops over 7-14 days. The 2-week point is when peak effect is visible, which is why we schedule a complimentary review at that time.
What's normal at 2 weeks: the lip looks subtly different — slightly fuller at rest, less inward-tucking when smiling. The change is intentional subtle; if you don't immediately see it, the comparison with your before photos at the review usually reveals the difference clearly.
What to discuss at review: how the result feels to you, whether there's any subtle asymmetry, whether the dose felt appropriate, what you'd like to think about for the next treatment.
Adjustments at review: small additions are sometimes possible if a slightly different dose distribution would optimise the result. This is one of the reasons the 2-week review exists. Patients who skip the review lose the opportunity to refine the plan with the clinician who has the clearest understanding of their specific case.
What's normal and what's not — recognising symptoms
Normal in the first 24 hours: pinpoint redness at injection sites · mild swelling that resolves by next morning · brief slight tenderness · slight altered sensation when speaking · feeling like "something is different" without being able to specify what.
Normal in days 2-14: gradual development of visible effect · awareness of lip behaviour being slightly different · adapting to the new lip-position-at-rest sensation.
Normal at 2 weeks: peak effect visible · subtle change from baseline · the result feeling "settled."
Not normal — contact the clinic:
- Significant swelling or asymmetry beyond what's expected in the first 24 hours
- Pain rather than mild tenderness
- Signs of infection — spreading redness, heat, discharge, fever
- Persistent altered sensation that doesn't resolve within a week
- Smile asymmetry that's still present after the 2-week settling window
- Difficulty articulating sounds that doesn't resolve within 1-2 weeks
- Any persistent effect on lip closure or function
Urgent — seek medical attention immediately:
- Hives or generalised rash (allergic reaction)
- Breathing difficulty
- Vision changes
- Severe headache
- Any neurological symptoms
These rare events warrant immediate medical attention and the clinic should be informed as soon as possible.

Specific patient questions answered
Can I drink alcohol after a lip flip?
Avoid alcohol for 24 hours after treatment. Alcohol's effects on circulation and potential interaction with mild bruising/swelling make day 1 a reasonable abstention window. From day 2 onwards, normal consumption is fine.
Can I eat normally after a lip flip?
Yes. Most patients eat normally immediately after treatment. The lips function normally for eating. Just avoid using a straw or significant sucking actions for the first 4 hours.
Can I kiss after a lip flip?
After the 4-hour settlement window, yes. Some patients prefer to wait until the next day; that's also fine. Kissing doesn't disrupt the treatment after the first 4 hours.
Can I sleep on my side after a lip flip?
Back-sleeping is best for the first night (4-12 hours) to avoid pressing the treated area into a pillow. From night 2 onwards, normal side-sleeping is fine.
Can I exercise after a lip flip?
Avoid vigorous exercise for 4 hours, then heat-exposure exercise (hot yoga, sauna) for 24 hours. From day 2 onwards, normal exercise is fine including weight training and cardio.
Can I wear makeup after a lip flip?
Avoid lip makeup for the first 4 hours; eye makeup is fine immediately. From day 1 onwards, all makeup is fine.
How long does lip flip swelling last?
Typically pinpoint swelling at injection sites resolves within 24 hours. Significant lip-area swelling beyond a few small injection points is uncommon with the small lip flip dose. Bruising at injection points (if any) resolves within 3-7 days.
When does a lip flip start to work?
Effect begins to develop around days 3-5 and peaks around 14 days. The first week typically shows subtle changes building gradually; the second week reveals the full effect.
When can I drink hot drinks after a lip flip?
After the 4-hour settlement window, hot drinks are fine. Just avoid the temptation to use significantly hotter-than-normal drinks during the first day; lukewarm temperatures during the initial settling period are easier on the system.
Can I have other aesthetic treatments at the same time?
Lip flip can be combined with upper-face muscle-relaxing treatment (forehead, frown lines, crow's feet) on the same day. Lip filler is better staged — filler first, lip flip 2 weeks later, or lip flip first and filler 2 weeks later. Energy-based treatments (laser, ultrasound) should be deferred for 2 weeks. Discuss combination at consultation.
What to bring to the 2-week review
The complimentary 2-week review is part of the lip flip service and exists to optimise your outcome. To get the most from it:
- The before photos from your treatment day (we keep these in your record; reviewing them together helps you see the change)
- Any specific observations about how the lip felt or functioned during the development window
- Photos of yourself from week 1 if useful for tracking development
- Questions about what to plan for the next treatment
The review takes 15-20 minutes and includes assessment of the result, photography for ongoing record, discussion of any small adjustments, and planning for the next treatment if you want continued effect.
The maintenance pattern after the first treatment
Most patients booking continuous lip flip maintenance plan every 8-10 weeks; our lip flip cost guide explains the pricing. The orbicularis oris muscle's natural metabolism of the medicine makes 6-10 weeks the typical effective window; some patients hold effect a little longer, some need treatment a little sooner.
The maintenance pattern is part of the consultation discussion at the 2-week review and at each subsequent appointment. Some patients book the next treatment at the 2-week review with a flexible date; some prefer to assess when they're noticing the effect fading and book then. Either approach works.

Frequently asked questions
What's the most important rule of lip flip aftercare?
The 4-hour rules — stay upright, no vigorous exercise, no pressure on the lip area, no facials, no straws. These protect the precision of the result and reduce the chance of avoidable spread effects.
How long until I see the lip flip working?
Effect begins to develop around days 3-5 and peaks around 14 days. Subtle changes build gradually through the first week; the second week reveals the full effect. The 2-week complimentary review is when peak effect is assessed.
How long does lip flip aftercare matter for?
Strict aftercare for the first 4-24 hours. Lighter considerations for the first week. After 2 weeks (peak effect), no specific aftercare restrictions remain.
Can lip flip aftercare affect how long the result lasts?
Marginally. Heavy exercise and high heat exposure in the early days can theoretically affect how the medicine settles and how long the effect lasts, but the difference is small. The bigger factor in result longevity is the individual patient's metabolism and muscle activity pattern.
What if I accidentally violated an aftercare rule?
Most aftercare rule violations have minor consequences. If you accidentally lay flat for an hour, ran a workout, or had a sauna in the first day — note it for the 2-week review discussion but don't panic. Significant deviation that affects the outcome usually involves multiple rule violations during the critical 4-hour window. One small accident rarely produces a noticeable effect on the result.
