What this page covers
The detailed, technical explanation of lip flip injection technique — where injections go, dose distribution, and why placement matters.
The standard lip flip technique — 3-4 point distribution
Two points on either side of the philtrum, 2-4mm lateral to the philtral columns just below the vermilion border. One or two central points at the midpoint. Optional lateral points where dynamics warrant. Total dose typically 2-6 units, divided across these points and calibrated to the patient's muscle activity and anatomy.
Why the vermilion border matters
The vermilion border — where lip tissue meets facial skin — is a reliable visible landmark accessing the upper orbicularis oris where the central pull is generated. The approach avoids deeper labial arteries while staying close enough to affect the target, relying on the anatomical literacy dental training provides for the perioral region.
Dose calibration
The 2-6 unit range reflects real variation in muscle activity: resting tone, dynamic activity during smile, visible muscle development, treatment history, and occupational risk tolerance (voice users, singers, wind instrument players may need lower doses).
Modified techniques for specific concerns
Gummy smile combination — combines with treatment to lip-elevator muscles, higher total dose. Asymmetry-corrective — dose distributed unevenly to correct pre-existing asymmetry without over-correcting. Lower-lip combination — additional lower-lip points, kept modest to avoid speech effects. Combination with filler — placement adjusted for how filler has changed lip position.
Why placement precision matters
Medicine spreads roughly 1-1.5cm from the injection site. Imprecise placement risks smile asymmetry, lip seal weakness (drooling, articulation issues), a downturned appearance, or inadequate effect if too superficial or lateral.
The injection sensation
Optional topical numbing, cleansing, marking, then a series of small injections (30-32 gauge needle) each a brief sharp pinch, 5-10 seconds. Whole procedure 5-10 minutes; no downtime.
Why outcomes vary across clinicians
Anatomical assessment quality, placement precision, dose calibration, and consultation depth all vary between clinicians — which is why prescriber status, training depth, and anatomical foundation matter more than the treatment name alone.
Frequently asked questions
How many injection points in a lip flip? Typically 3-4 along the vermilion border, distributed based on muscle activity.
What does the injection feel like? A brief sharp pinch at each point; the whole procedure takes 5-10 minutes.

How much anti-wrinkle product is used? Typically 2-6 units — one of the smallest doses in anti-wrinkle treatment.
Why does placement precision matter? Imprecise placement can catch unintended muscle fibres, producing asymmetry or lip seal weakness.
Can the technique be adjusted for asymmetry? Yes, with dose distributed unevenly to correct without over-correcting.
