Leg thread vein removal is a question most people sit on for years. The small red and blue lines on the thighs, behind the knees or around the ankles are rarely a medical problem, yet they can make shorts, swimwear or bare legs feel like something to plan around. This guide sets out what thread veins are, which ones respond to treatment, how we treat them at 222 Essex Road, what it costs and where the honest limits lie, so you can decide whether it suits you before you book anything.
Thread veins, also called spider veins or telangiectasia, are small dilated blood vessels sitting just beneath the skin. On the legs they usually measure less than a millimetre or two across and appear as fine red lines, purple-blue branches or small starburst clusters. They are different from varicose veins, which are larger, raised, rope-like veins caused by faulty valves in the deeper venous system.
Several factors make leg thread veins more likely. Family history is the strongest. Hormonal change plays a part, which is why they often appear or worsen during pregnancy, with hormonal contraception or around the menopause. Long hours standing or sitting, rising body weight, previous injury to the leg and, simply, age all add to the picture. Sun exposure matters more on the face than on the legs, but it still has some effect on exposed skin. For most people leg thread veins are a cosmetic concern. That said, a cluster of thread veins can occasionally be the visible surface of an underlying problem with the deeper veins, and that changes what the right first step is. We come back to this below.
Laser and light treatment suits fine red and blue thread veins on the legs in people who are otherwise well. The best results are usually seen in vessels that are fine, flat and clearly visible, in skin that has not been recently tanned.
Treatment is not the right starting point for everyone. We do not treat varicose veins at the clinic. If you have bulging or rope-like veins, legs that are swollen, aching or painful, skin that has become discoloured, hardened or itchy around the ankles, or any ulcer or open wound on the leg, please see your GP first. These signs can point to venous disease that needs assessment, often with a duplex ultrasound scan, and a referral to a vascular specialist. Treating the surface thread veins without addressing an underlying cause tends to give poor and short-lived results.
We also do not treat during pregnancy. Thread veins that appear in pregnancy sometimes settle by themselves in the months after the birth, so it is sensible to wait and see what remains. Recent sun exposure or a tan, including fake tan, means treatment is postponed. A history of keloid scarring, certain light-sensitising medicines, active skin infection over the area or a bleeding disorder all need discussion at consultation, and may mean treatment is not appropriate.
Darker skin can be treated, but it needs particular care. The pigment in the skin also absorbs light, so the device, wavelength and settings must be chosen carefully, and a patch test is especially important. In some cases the safest advice is not to proceed, and we will say so.
Both devices rely on the same principle. Light of a chosen wavelength is absorbed by the haemoglobin in the blood inside the vessel. That light is converted to heat, which damages the vessel wall. Over the following weeks the body reabsorbs the treated vessel, and it fades from view. The surrounding skin is protected by cooling and by careful control of energy and pulse length.

Most people describe the sensation as a quick, hot snap against the skin, similar to a flicked elastic band. It is brief, the treatment head is cooled, and a session on the legs typically takes between 15 and 45 minutes depending on how many areas are treated.
Leg thread veins usually need 2 to 4 sessions, spaced 6 to 8 weeks apart. That interval matters: the body needs time to clear the treated vessels before we can judge what remains and treat it. Fine red and blue veins respond best. Larger blue vessels, and veins fed by a deeper source, respond less predictably.
Straight after a session, treated veins may look darker, slightly raised or bruised. They then fade gradually over several weeks. Leg vessels tend to take longer to clear than facial ones, so the full result of each session is judged at the next appointment rather than in the first few days.
Results vary from person to person, and we do not promise complete clearance. A realistic aim is a clear reduction in the visibility of the treated veins. Vessels that have been successfully treated do not usually return, but the tendency that produced them remains, so new thread veins can form over time. Some patients choose an occasional top-up session in later years.
There is no real downtime and most people return to work the same day. Expect some redness and mild swelling along the treated vessels for a day or two, and possibly some bruising or darkening that settles over the following weeks.

Laser and IPL treatment of leg veins is widely used and generally well tolerated, but no treatment is without risk. Common, short-lived effects include redness, swelling, bruising and a temporary darkening of the treated vessel. Less common effects include blistering, crusting, temporary darkening of the skin (hyperpigmentation), lightening of the skin (hypopigmentation) and, rarely, scarring. Pigment change is more likely in darker skin and in skin that has been in the sun, which is why skin assessment, a patch test where needed and careful aftercare matter. These risks are discussed in full, and in writing, before you consent to treatment.
