When extraction is genuinely needed
Severe crowding beyond expansion capacity, specific anatomical situations (narrow palate, ankylosed teeth), certain bite corrections, or severely problematic teeth. The philosophy is non-extraction-preferred, not non-extraction-only.

Why Diamond II Provider matters here
