Two routes to the same goal

Onda and CoolSculpting are both non-surgical treatments for pockets of fat that persist despite a steady diet and regular exercise, most often on the lower abdomen, flanks, inner thighs and upper arms. Neither is a weight-loss treatment. Each injures a proportion of the fat cells in the treated area, and the body then clears those cells gradually over the following weeks.

  • Onda (Onda Coolwaves) delivers microwave energy at a frequency absorbed mainly by fat. The skin surface is cooled throughout, so heat builds in the fat layer rather than in the skin. It is usually given as a course of four sessions.
  • CoolSculpting is a branded form of cryolipolysis. An applicator cools the fat to a temperature that injures fat cells while a protective pad spares the skin. Each area is treated in one or more cycles, sometimes repeated.

The key difference

On results, the two are broadly comparable: a gradual, moderate reduction in a defined area, and results vary from person to person. The difference that shaped our choice is a rare complication called paradoxical adipose hyperplasia (PAH), in which a cold-treated area becomes firmer and larger rather than smaller. It is uncommon, but it does not settle on its own and is usually corrected surgically. PAH is specific to cold-based treatment, which is why we chose Onda Coolwaves for our body contouring service and do not offer CoolSculpting. How to read this page

The table compares the two factor by factor. The sections that follow set out who each option tends to suit, then our clinical recommendation and the questions patients ask most. CoolSculpting is a legitimate, widely used treatment, and we describe it factually here; if it suits you better, we will tell you. Both Onda and CoolSculpting reduce stubborn, localised fat without surgery, and both need time before results show. Onda uses targeted microwave energy; CoolSculpting uses controlled cooling, known as cryolipolysis. Results are broadly comparable. The practical difference is that cooling carries a rare complication, paradoxical adipose hyperplasia, which is specific to cold-based treatment. We offer Onda at our Islington clinic; we do not offer CoolSculpting.

Which one is right for you

Onda tends to suit you if you are at or near a weight you can maintain and there is a defined area of fat that has not responded to sensible eating and regular movement. Typical areas are the lower abdomen, flanks, inner thighs, upper arms and the area around the bra line.

  • You would rather avoid the rare cold-specific risk of paradoxical adipose hyperplasia.
  • You are comfortable with a course of treatment and with results that build gradually rather than appearing at once.
  • You also notice the dimpled texture of cellulite in the same area, where Onda can make a modest difference.
  • You want fat reduction as one part of a wider plan, alongside personal training or nutritional support.

That last point matters to us. The ONDA course of four includes an online personal training session, because results last best on a stable frame. Where the main concern turns out to be loose skin rather than fat, we will say so and point you towards laser skin tightening instead.

CoolSculpting may suit you if you have a defined, pinchable pocket of fat, a stable weight and a clear preference for cold-based treatment. Cryolipolysis has been in wide use for many years, it is offered by many reputable clinics, and most people treated with it experience only temporary effects such as numbness and tenderness.

  • You have already been assessed at a clinic you trust that offers it, and you are happy in their care.
  • You have weighed the rare risk of paradoxical adipose hyperplasia and are comfortable with it.
  • You prefer fewer, longer appointments per area to a course of shorter sessions.

It is not suitable if you have a cold-related condition such as cryoglobulinaemia or cold agglutinin disease, or if you are pregnant. As with Onda, it is not a weight-loss treatment.

We do not offer CoolSculpting, so we cannot quote for it. If, after a consultation, cold-based treatment is clearly your preference, we will say so plainly and suggest you see an established provider. Our CoolSculpting alternative page explains what we offer instead.

In my practice, the choice between heat and cold came down to one question: if two treatments give broadly similar results, which carries fewer avoidable risks? Paradoxical adipose hyperplasia is rare, but when it happens it is persistent and usually needs surgery to correct. Onda reaches a comparable result without that specific risk, so it is the device we chose. That is a clinical judgement for our patients, not a criticism of clinics that use cryolipolysis well.

I would also say that fat reduction is rarely the whole answer. Before recommending a course, we look at whether the concern is fat, loose skin, or both, and whether weight is stable. Sometimes the honest plan pairs Onda with skin tightening; sometimes it is a smaller treatment such as fat-dissolving injections; occasionally it is no treatment at all until other things settle. Results vary, and we explain what is realistic before you commit to anything.

  • Is Onda a weight-loss treatment?

    No. Onda reduces fat volume in a specific treated area; it does not meaningfully change your weight and is not a substitute for diet, movement or medical weight management. It is a contouring treatment for defined pockets that persist despite those things, and it works best when your weight is stable.

  • Why do you offer Onda rather than CoolSculpting?

    Because of paradoxical adipose hyperplasia, a rare complication of cold-based fat reduction in which the treated area enlarges rather than shrinks. It does not resolve on its own and is usually corrected surgically. Onda works by heat, so that specific risk does not apply. This is a clinical judgement for our practice, not a claim that CoolSculpting is unsafe.

  • Is Onda as effective as CoolSculpting?

    Broadly, yes. Both produce a gradual, moderate reduction in fat in the treated area, and both need several weeks before the change is visible. The mechanisms differ, heat against cold, and individual results vary with the area, the amount of fat and how stable your weight is afterwards.

  • How many Onda sessions will I need, and what does it cost?

    Most people have a course of four sessions, a few weeks apart, though some areas need more. Onda is £300 per session, or £1,020 for a course of four, which includes an online personal training session. We confirm the plan and the full cost in writing after your consultation.

  • Does Onda hurt, and is there downtime?

    Most patients describe a building warmth with brief moments of more intense heat, and the skin is cooled throughout. There is usually little or no downtime; temporary redness, warmth or tenderness in the area is normal and settles. Most people return to normal activity the same day.

  • When will I see results, and how long do they last?

    The treated fat cells are cleared gradually, so change builds over the weeks following each session and across the course. Cleared fat cells do not return, but the remaining cells can still enlarge with weight gain, which is why steady eating and activity patterns matter for lasting results.

  • Who carries out Onda at the clinic?

    Every course begins with an assessment with Dr Vishal Patel, who trained with Lynton Lasers in Onda and plans each treatment. Onda is a device treatment rather than a prescription medicine, but careful case selection still matters: we check your medical history, the area and the type of tissue before recommending it.

  • Can Onda tighten loose skin?

    Onda can give a mild tightening effect and may improve the dimpled texture of cellulite, but it is primarily a fat-reduction treatment. If loose skin is your main concern, a dedicated skin-tightening approach is usually more appropriate, and we will say so at consultation rather than sell you the wrong course.
