Invisalign refinement is a second set of aligners issued during treatment to fine-tune teeth that haven't fully reached their planned positions. Around 30-40% of Invisalign cases need refinement — particularly more complex cases. At Dental & Wellness London, Express and Moderate each include one refinement, and the Unlimited plan covers refinements as the case requires and includes multi-year aligner cover (full terms and conditions apply).
The word that worries patients unnecessarily
"Refinement" sounds like something went wrong. It isn't — it's a planned-for stage reached by around a third of cases, and the technology is built to accommodate it.
What refinement actually is
A second (occasionally third) set of aligners closing the gap between where teeth actually are at the end of the initial series and where ClinCheck predicted. Teeth are living tissue, and small individual variations mean some movements don't quite complete on the first pass.

How common is refinement
Around 30-40% of cases overall. Express: 10-15%. Moderate: 30-40%. Unlimited: 40-50%, due to greater complexity. The Unlimited plan is designed for extended refinement over years.
Why teeth don't always track the plan
Biological variation, compliance gaps, attachment performance, complex movements (root torque, intrusion, severe rotations), anatomy variability, and existing restorations moving differently from natural enamel. None of these is a failure.
How the refinement process works
Assessment at the end of the initial series → decision to refine if movements are incomplete → new 3D scan → refinement ClinCheck (often 5-12 aligners) → refinement aligners on the same 22-hour schedule → final assessment → retainer phase. Typically adds 2-6 months.

What refinement costs
At DWL, Express and Moderate include one refinement and Unlimited covers refinements as the case requires, all set out in your written treatment plan before you start (see Invisalign cost). Other clinics may charge £400-£900 per round separately — worth asking explicitly at consultation whether refinement is included in the headline price.
Refinement vs relapse
Refinement happens during active treatment, completing the original plan. Relapse happens after treatment ends, when teeth drift back due to insufficient retainer wear (see Invisalign for orthodontic relapse) — needing a new case from the relapsed position, not a refinement.
When refinement isn't enough
Options include switching to fixed braces, accepting a sound functional result short of 100% of plan, combining with selective composite bonding (£175 simple, £350 complex, per tooth), or specialist referral for complex skeletal patterns.

Frequently asked questions
Does needing refinement mean Invisalign didn't work? No — it's a normal, planned-for part of modern treatment, reached by 30-40% of cases.
How long does refinement take? Simple refinement 6-8 weeks; more involved refinement 3-6 months.
Do I have to pay extra? What is included is set out in your written treatment plan before you start; other clinics vary.
Can I skip refinement if I'm happy? Yes, if the result is a stable functional outcome.
Will I need more than one round? Most need only one; 5-10% benefit from a second.
